COVID-19 Policy Acknowledgment

By clicking “Register”, you are acknowledging that an inherent risk of exposure to COVID-19, a variant, or any other illness exists in any public place where people are present. By attending any event hosted by Glazier Clinics, you and any guests voluntarily assume all risks related to exposure to COVID-19, a variant, or any other illness, and agree not to hold Glazier Clinics or any of its affiliates, directors, officers, employees, agents, attendees, partners, exhibitors, contractors, or volunteers liable for any such illness contracted during attendance at a clinic. You and any guests agree to adhere to any state and local COVID-19 regulations and policies as well as Centers for Disease Control and Prevention (CDC) guidelines regarding COVID-19. For any health and wellness related questions, refer to the latest guidance by the CDC regarding attending events.

Attendee Terms and Conditions

Any person, delegate, accompanying individual, student, media representative, speaker, partner or exhibitor who is present at a Glazier Clinics event (“Clinic”) agrees to the terms and conditions set forth below (the “Terms and Conditions”.) These Terms and Conditions apply to each person/school/organization (the “Attendee”) regardless of whether they have received a formal registration. Glazier Clinics reserves the right to render, in its sole and absolute discretion, all interpretations and decisions in connection with the Terms and Conditions and to establish further regulations binding upon the Attendee as may be deemed necessary for the general success and well-being of the Clinic and/or any person(s) affiliated therewith. Glazier Clinics’ decisions and interpretations shall be final in all cases.

Refund Policy for Clinic Registrations. All Clinic registration sales are final and nonrefundable. A registration may not be transferred to another individual. For more information, please contact info@glazierclinics.com.

Rules, Regulations, & Attendee Conduct. Attendee agrees to abide by all fire, safety, and health regulations of the local, city, state, hotel(s), and convention center(s) where the Clinic is held. Attendee must comply with all applicable federal, state, and local ordinances, as well as Glazier Clinic’s policies and procedures as set forth below. Attendees who violate such rules and/or who behave in an unsafe or careless manner while attending the Clinic may be asked to leave immediately and will be held responsible for their actions, whether criminal, legal or financial. The Attendee also acknowledges that such violations may result in the Attendee being removed from the Clinic and barred from returning without a refund of the Attendee’s registration fee. The Attendee likewise acknowledges and agrees that all attendees of the Clinic have the right to enjoy the Clinic equally. Illegal or disruptive behavior that interferes with the reasonable conduct of business at the Clinic, or any other attendee’s safety or comfort, including behavior that impedes or prevents accessibility services or requests, behavior that discriminates based on age, disability, race, ethnicity, gender, religion, political or institutional affiliation, or sexual orientation, and harassment of any kind, is strictly prohibited and may result in the Attendee being removed from the Clinic and barred from returning without a refund of the Attendee’s registration fee. Such violations may be reported to the appropriate authorities. If you witness a crime or have an emergency that requires the police, please call 911.

Indemnity. To the extent permitted by law, the Attendee agrees to protect, indemnify, defend, and hold harmless Glazier Clinics and its respective employees, officers, directors, and agents against all claims, losses, or damages to persons or property, governmental charges or fines, and costs (including reasonable attorney's fees) arising out of or connected to the actions or failure to act in accordance with the Terms and Conditions by the Attendee.

Alcohol & Drugs. Glazier Clinics supports all laws of the state and municipality where the Clinic is held in all matters concerning alcohol and other controlled substances. Alcoholic beverages may not be sold or consumed in the Clinic area (such as a meeting room) without prior approval from Glazier Clinics staff. Individuals who appear to be under the influence of alcohol or drugs and/or who’s behavior is disruptive or threatening to other attendees, will be asked to leave the Clinic (Also see Rules, Regulations, & Attendee Conduct.)

Smoking. Smoking or vaping of any kind is not permitted in the hotel or convention center where the Clinic is being held, the exhibit space, or the sleeping rooms unless specifically permitted in designated smoking areas or sleeping rooms.

Lost Registration Badge. The Attendee’s registration badge must be worn at all times during the Clinic. Access to the Clinic facilities will not be granted to those without proper credentials. Attendees not displaying their registration badge in the Clinic area will be asked to do so by Glazier staff. If an Attendee loses, misplaces, or forgets his or her registration badge, a replacement fee of $50.00 will be charged for the creation of a new one. Once the new registration is released to the Attendee, the lost registration will become invalid.

Modification of the Clinic Program. Glazier Clinics reserves the right to modify the program schedule without advanced notice.

Registration, Hotel, & Travel Accommodations. Registration includes admission to Glazier Clinics meetings, gatherings, and panels for the duration of the clinic period. Meals, lodging, and travel are not included. Hotel accommodations are subject to availability and will be allocated by the hotel on a first-come, first-served basis.

Salvatory Clause. If there is a determination that any part of the Terms and Conditions is unenforceable, ineffective or impracticable, the validity of the Terms and Conditions remaining is not impaired.

Digital & Other Recordings. Professional photographs, audio, and video may be captured by Glazier Clinics before and during the Clinic. Presenters and Clinic Attendees grant Glazier Clinics, its agents and representatives, permission to photograph and/or record them before and during Clinic events and to use such recordings in support of Glazier Clinics’ mission. Uses of records may include, but are not limited to, e-broadcasting on Glazier Clinics’ YouTube Channel, educational initiatives, promotional marketing, post-clinic reporting, and Glazier Clinics’ podcast series.

Photography or Recording Usage by Attendees. Photography, screenshots, audio recordings, and video recordings are not permitted without the prior written consent of Glazier Clinics, made in advance of the Clinic to info@glazierclinics.com. Glazier Clinics requires that a copy of all published material be submitted to info@glazierclinics.com.

Speakers/Presenters Consent to Recording. Speakers/Presenters provide consent for Glazier Clinics to record presentations in audio and/or visual form and grant Glazier Clinics a royalty-free license to use, reproduce, and distribute the recordings and accompanying materials. Speakers/Presenters also understand that while they retain the rights to their presentation materials (handouts, slides, etc.) and the content of the presentation, Glazier Clinics is the sole copyright owner of any recording and can distribute it, along with any supporting materials in any way.

Copyright & Trademark Permissions. Speakers/Presenters agree, to the best of their knowledge, that their presentations do not violate any proprietary or personal rights of others (including any copyright, trademark, and privacy rights), are factually accurate, and contain nothing defamatory or otherwise unlawful. Speakers/Presenters have full authority to enter into this agreement and have obtained all necessary permissions or licenses from any individuals or organizations whose material is included or used in a presentation. Speakers/Presenters warrant their materials are original or properly licensed and agree to defend and indemnify Glazier Clinics against any potential copyright infringement or other claims. Speakers/Presenters additionally grant license to Glazier Clinics to all provided content.

Attendance Restriction. Children and "prospective student-athletes" as defined by the NCAA, are not allowed to be in attendance at Glazier Clinics, including but not limited to meeting rooms, exhibit areas, or any other areas where Glazier Clinics events are being conducted.

Force Majeure. Attendance at the Clinic is subject to acts of God, accidents, riots, wars, terrorist acts, epidemics, pandemics, local disease outbreaks, public health emergencies, quarantines, civil commotions, breakdown of communication facilities, breakdown of the web host, breakdown of the internet service provider, natural catastrophes, government acts or omissions, changes in laws or regulations, national strikes, fire, explosion, general lack of availability of raw materials or energy, disasters, or other emergencies, curtailment of transportation, imperiling weather or any other such emergencies severely restricting or preventing Glazier Clinics from holding the Clinic, or attendees from attending the Clinic. The Clinic may be cancelled without liability on the part of Glazier Clinics, for any one or more of such reasons and attendees will be notified by a message from Glazier Clinics on its appropriate web page or using any other method Glazier Clinics determines to be most effective and practicable in the situation.

Enforcement. Enforcement of the rules set forth in these Terms and Conditions is solely at Glazier Clinics' discretion, and failure to enforce such rules in some instances does not constitute a waiver of Glazier Clinics' right to enforce such rules in other instances.

Cancellation. In the event that the Clinic cannot be held or is postponed due to events beyond the control of Glazier Clinics, Glazier Clinics shall not be liable to the Attendee for any damages, costs, or losses incurred, such as transportation costs, accommodations costs, healthcare costs, or financial losses.
